We are redesigning our service to move away from the traditional “one size fits all” model, which often undervalues individuals and fosters dependency.
Our new approach centers on frontline colleagues working within their local communities, empowered to focus on what truly matters for the people they know so well.
While this is a work in progress, the initial evidence is very encouraging.
Currently, our Home Care service for older people is available only in Arfon, Gwynedd. However, the innovative model we are developing can serve as a blueprint for home care across Wales and beyond.
